This MS Word template is a single page that can be customized for each show.

Included are five blocks in table format for:

• Name of show, opening/closing date, names of director, stage manager and assistant stage manger.
• Running times for each act and intermission
• General issues such as latecomer seating hold, actors entering through the house, ASL interpreters scheduled, obstructed views, etc.
• Effects, such as offstage and onstage gunshots, fog/smoke, strobe lights, other loud sounds and effects.
• Content, such as profanity, nudity, smoking/drinking onstage, depictions of drug use or violence, sexual/suggestive content.
• Plus space for additional notes/information.
